NEW PATIENTS

New patients are welcome from within the practice area.

If you are registering for the first time in Northern Ireland or are returning to Northern Ireland after a period away, you will be asked to complete Form HSCR1.  These can be requested from the practice by calling at the practice or telephoning 02892666266

OR

registration form HS200 - application form for patients who are presently from Britain.

All new patients will also be asked to provide:

  • Photographic ID (copy of passport or driving license)
  • Proof of address (copy of a utility bill or recent bank statement)
  • Visa (if applicable)

We ask all new patients to attend for a new patient medical – this enables us to obtain relevant information while waiting for your previous medical records to reach us. It also gives you the opportunity to meet one of the practice doctors and have a health check.

If you have lost your medical card contact Business Support Services (telephone number 0300 555 0113) for a replacement

All patients on the following drugs will have a reduction of these discussed at their new patient medical.

  • Diazepam
  • Tramadol
  • Pregabalin/Gabapentin
  • Dihydrocodeine/Codeine
  • All Sleeping tablets
